AgeWell PACE is a program of all -inclusive care for the elderly. AgeWell provides wrap around healthcare services to qualifying seniors, including all primary care onsite long side dentistry, podiatry, mental health services, optometry and rehab therapy. Included in the services are transportation to and from your home to the center, access to the adult day program where we host all kinds of fun activities and movement classes. Unskilled homecare is also provided for those participants who need it.
